Why Is a Hearing Test Essential Before You Get a Hearing Aid?

This year's World Hearing Day, March 3rd, 2024, focuses on overcoming the challenges posed by societal misperceptions and stigmatizing mindsets through awareness-raising and information-sharing. In the spirit of World Hearing Day and Orka's empowering campaign, "You are full of possibilities," we had the chance to sit down with Xinke Liu, Co-Founder and Chief Communications Officer at Orka, for a conversation.

Since co-founding Orka in Silicon Valley in 2019, Xinke Liu and the team have been on a mission to empower people with hearing loss to communicate freely and engage fully in life's significant moments. Xinke is a person of many facets—enthusiastic, determined, a sports lover, and an entrepreneur. But one thing that might not immediately come to people's mind is that she's been navigating life with severe hearing loss since her teenage years.

After her first hearing test at the age of 15, Xinke was diagnosed with profoundly severe hearing loss in the left ear and severe hearing loss in the right ear. Like many, Xinke believed that there were many ways to compensate for her hearing loss. After all, it didn't cause her physical pain, and she could still navigate her daily tasks. 

She filled the silence with lip-reading, a skill that served her well until she moved to the United States for her studies. In this new world, English—the language that wasn't her mother tongue—became her primary mode of communication, and lip reading was no longer reliable. 

**"When you can't communicate with people, you would start telling yourself lies. At school, I was isolating myself and saying 'Oh, I'm an introvert, and I like being on my own,"' but that's not true. It's just a fat lie I liked to tell myself,"** Xinke had put herself in a box, limiting herself to only the things she thought she could do.

7 years after her first hearing loss diagnosis, Xinke got her first cochlear implant (CI). The world around her, once muffled and distant, burst into life with sound. She still vividly remembers the first time she walked out of the doctor's office with CI:" Then magic happened. I activated my CI for the first time and took a taxi. Suddenly, I noticed the advertisements in the taxi playing, and I was thrilled! I started talking to the taxi driver about this 'magic' thing that just happened."

**"I have to say, even though my personality is like accepting everything that happens to me, I did regret that I waited so long before having my first CI surgery," Xinke confessed.**

Xinke embraced her new world of sound with an open heart and a curious mind, and she is now a sports enthusiast who never shies away from new challenges: whether it's skiing down snowy slopes, riding a wakeboard, or galloping on horseback, Xinke takes on each adventure with the same zest.

"I get asked questions about my cochlear implants all the time," Xinke says with a smile, " like 'Do you wear your CI while skiing? What about surfing? And do you have to take it off at airport security?' I understand the curiosity about things you don't usually come across, and I know that some hearing aid or CI users can feel anxious about these things, too. But honestly, my experience has shown me that it's not as complicated as it might seem."

For Xinke, it's all about personal comfort: "If you feel like wearing your hearing aids or CI while skiing, go for it. If not, that's fine too. Personally, I always wear my CI when hitting the slopes. I make sure to secure it with a headband because, let's face it, losing it would be a disaster. Sometimes it can be tricky fitting it under a ski helmet, so I just get a bigger helmet. I will say though, my helmet has a big 'XL' on it. My brother laughed at it so hard."

"When I do wake surfing, well, I can't wear my CI. It does create issues when you're trying to understand the surfing instructor. But I just ask him to get a bit more creative in how he teaches—maybe use more body language. **Honestly, skipping the CI for wake surfing? Does it really impact your performance? Not at all! "**

"I want to emphasize that it's not just about 'accepting hearing loss' as a concept. Step one is to truly grasp that hearing loss is totally normal, and can happen to anyone. I believe a better approach than just trying to 'accept hearing loss' is realizing that even if you do have hearing loss, it's not as bad as you might have imagined in your head. There are still endless possibilities for what you can achieve."

Xinke has a motto: **"I always say: 'Wear the clothes, don't let the clothes wear you.' It's the same with hearing devices—they're just tools. You shouldn't let them take over your life or who you are."**

Xinke Liu: "I always say: 'Wear the clothes, don't let the clothes wear you.' It's the same with hearing devices—they're just tools.

In 2018 I was in my third year designing products for Apple. I could envision a future in Cupertino. Who knows, maybe I could be a part of the next iPad or Apple Watch.
Then, over the holidays visited my grandma. Her hearing had been in slow decline for decades, but I noticed she was struggling to stay involved in conversations. I figured I'd reboot her hearing aids and fix the issue - after all, technology is what I do.
After an hour of tinkering, I realized this wasn't a bug; her hearing aids simply were not capable of managing background noise, and the design was, well, not Apple.
I couldn't stop thinking about it. Why on earth were there not hearing aids with modern consumer-tech quality design? And how could we fix this background noise issue?
Before long, my thoughts turned to sketches, and eventually, I started talking to my friends. Xinke had a cochlear implant and a passion for making the space more accessible. Chauncey was on the AI team at Facebook, and Linkai was at Siemens, a company with a long history in hearing health. Before long, we were pulling late nights, building prototypes in my kitchen.

Over the next few years, our prototypes and the technology inside matured. We won awards and started seeing impressive results in background noise:
We were recognized with the <a href="https://www.hiorka.com/blogPage/orka-one-won-2021-reddot-award" target="__black"><Typography variant="LINK" fontWeight="400">Red Dot Award for Product Design</Typography></a>, and the honor of being a CES InnovationAwards honoree for our innovative technology and human-centered design. Good progress. But now we had a big decision to make.

I have had several different models that were made by different major manufacturers. I tried the Bluetooth® features with the clumsy hang around the neck interfaces that never worked. Although the hearing aids helped with hearing sounds I personally never really noticed any improvements on clarity of voice communication. Some people also call this certain type a power dome because they deliver the best sound amplification. The double-layered design offers the best seal so that the least amount of sounds can enter your ear canal without being processed by hearing aids. That said, whistling can be reduced to the largest extent when you pair this type with your hearing aids. And they are recommended for people with a greater need for sound enhancement.

Whistling/feedback is probably one of the most frustrating yet common issues concerning hearing aid users. It's important to note that while vented domes can be a major cause for whistling, other factors such as earwax build-up and excessively high volume can trigger feedback too. When there is constant whistling with our hearing aids, it's best to check your domes for earwax blockade and damages, then change to a new one where needed . If the situation does not improve, switching to an occlusive type is always recommended.

Besides a match of type and regular replacement, proper wearing of the domes matters as well. A loose dome in the ear leaves a lot of room for unwanted sounds to sneak in and out, thus causing significant feedback. Always make sure we try out every single size under each type until we find the best fit.

Fun fact: whistling can occur particularly when we are chewing food, where the inner shape of our ear canals change alongside our jaw movement. This can further loosen the domes and widen the gap. Next time when trying out domes, drop your jaw and see whether the dome feels secure in the ear canal.

For many first-time users, starting with closed or double domes is not going to be a satisfying experience. In addition to a discomfort of being stuffed in the ears, there is an occlusion effect that often comes into play where body-conducted sounds like our own voice gets trapped in the blocked ear canal, resulting in a hollow and bass-like sounding effect, aka occlusion effect.

This explains why audiologists suggest trying open-type domes to start for users with relatively mild hearing loss. Most of the hearing aids in the market can only stream audio directly from IOS system devices like iPhone, but not from Android devices and other standard Bluetooth® devices. FDA specifically emphasizes the importance of hearing aids safety to users in the proposal as well as standardized marketing of hearing aids will be established. The sale of OTC hearing aids is strictly forbidden to or for people younger than 18. FDA would also define OTC hearing aids as misbranding unless its labeling bears adequate directions for use.

The point of the audiogram is to determine where your ability to hear falls on the typical range of sound levels.
 
In most cases, the “normal” range will be from zero to twenty decibels on the vertical axis. Results falling outside of this range could be a sign that you are suffering from some degree of Hearing loss.

 

**Types of Hearing Loss:**
1. **Conductive Hearing Loss:** This type of hearing loss is characterized by a discrepancy between bone conduction scores, which are normal, and air conduction scores, which reveal a hearing loss. In other words, an audiogram would show a normal ability to hear when sound bypasses the outer and middle ear (via bone conduction), but a decreased ability when sound travels the usual path through the air and these parts of the ear (air conduction). This indicates a problem with sound transmission through the outer or middle ear to the inner ear.
 
2. **Sensorineural Hearing Loss:** This is a type of hearing loss where both air and bone conduction scores reveal a hearing deficit. An audiogram would show a similar decrease in hearing ability for both types of conduction, without a significant gap between the two. This typically points to an issue in the inner ear or auditory nerve, where the conversion of sound to electrical signals occurs.
 
3. **Mixed Hearing Loss:** This form of hearing loss is represented by a decrease in hearing ability for both bone and air conduction, with the loss being greater for air conduction. An audiogram would show a decrease in hearing ability when sound is transmitted either through the bone or through the air, with the air conduction loss being more pronounced. This suggests a combination of conductive and sensorineural hearing loss.

An audiogram is a graph that shows the softest sounds a person can hear at different frequencies. The term 'speech banana' comes from the banana-like shape that the sounds of human speech form on this graph.
 
The low sound frequencies, ranging from 125 Hz to 1000 Hz, are primarily responsible for a person’s interpretation of the volume of speech. These frequencies often correspond to the vowels and some consonants in speech, which carry the bulk of the voice's power and volume.
 
In contrast, high sound frequencies are responsible for the clarity and sharpness of speech. These frequencies, typically above 1000 Hz, are where many consonant sounds fall, particularly those that are unvoiced like “f”, “ph”, “th”, “s” and “t”. These sounds are essential for distinguishing between similar words, and they are often the hardest to hear for individuals with high-frequency hearing loss.
 
Many people with high-frequency losses express a common sentiment: “My hearing is good, people just sound like they are mumbling.” This is because while they can hear the volume of speech (low frequencies), they struggle with the clarity (high frequencies). The speech banana concept helps illustrate this phenomenon, showing how different speech sounds fall within different frequency and volume ranges, and how hearing loss in certain ranges can impact speech comprehension.
 
**So what now?**

**ITE Hearing Aids (In-the-Ear)**

An in-the-ear (ITE) hearing aid is a customized device that fits around the outside of your ear. The ITE hearing aid is a larger device, so short battery life shouldn’t be an issue. It has features that smaller styles don’t have such as volume control. It might pick up more wind noise than smaller hearing aids. They are also made in two styles — one that fits all of the outer ear called full shell and one that only fits the lower half (half shell). They are suitable for people with mild to severe hear loss.

**BTE Hearing Aids (Behind-the-Ear)**

A behind-the-ear (BTE) device is the most common style in the world. It’s the biggest hearing aid available so the device provides the user with longer battery life and more power. The BTE style is the least discreet out of all hearing aids as it attaches behind your ear. It is available in different sizes and the new mini designs can be customized to be barely visible.

**RIC and RITE Hearing Aids (Receiver-in-canal or receiver-in-the-ear)**

A receiver-in-canal (RIC) and receiver-in-the-ear (RITE) hearing aids appear similar to the BTE devices with the receiver fitted in the ear canal. It is a type of hearing aid that is known to be suitable for most hearing loss situations. The difference is that a small wire instead of a tube connects it to the speaker or receiver behind the ear. They are smaller than the typical BTE devices so are more discreet. It also comes with a bigger battery, which lasts longer. However, these devices are more prone to clogging from earwax.
